Nursing Care
Coordinates admission, transfer and discharge processes.
Informs nursing personnel of new admissions, their expected time of arrival, room assignment, etc.
Ensures that rooms are ready for new admissions.
Considers problems with vision, hearing, mobility, etc. when communicating or working with residents.
Participates in the orientation of new residents/family members to the facility.
Communicates with physicians appropriately.
Reviews the resident's chart for specific treatments, medication orders, diets, etc., as necessary.
Requisitions and arranges for diagnostic and therapeutic services, as ordered by the physician, and in accordance with established procedures.
Consults with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.
Notifies the resident's attending physician and next-of-kin when there's a change in the resident's condition or the resident is involved in an accident or incident.
Makes clinical rounds on unit on own daily and with MD as necessary.
Implements and maintains established nursing objectives and standards.

Administers professional services such as; catheterizations, tube feedings, suctioning, applying and changing dressings/bandages, colostomy and drainage bags, care for the dead and dying, etc., as required.
Monitors for lab specimens being obtained and results returned.
Obtains and records vital signs as necessary.
Assists in arranging consults/follow-up MD appt's as necessary.
Enforces unit-specific patient care routines, i.e. weights, temperatures, supplements, trays, Intakes & Outputs, etc.
Participates in evaluation, development and documentation of restorative programs.
Demonstrates correct use of Pyxsis system.
Makes periodic checks to ensure that prescribed treatments are being administered by the staff.
Notifies family members of resident changes in condition.
Meets with residents and/or family members, as necessary. This may be through family ICC; rehab meetings, etc.
Informs family members of the death of a resident and calls funeral home when requested by family. Ensures post-mortem care is delivered.
Monitors unit for prompt response to resident call lights by staff.

Care Plan and Assessment Functions
Assists in performing resident admission assessments as well as discharge planning for the residents.
Participates in development and communication of residents' short term plan of care as well as long term plan of care to all team members.

Charting and Documentation Functions
Performs routine charting duties as required and in accordance with established regulatory, professional, and Maple Knoll Village policies and procedures for charting and documentation.
Uses only approved abbreviations.
Receives telephone orders from physicians and records on the Physician's Order Form.
Transcribes physicians' orders to resident charts, medication record, treatment/care plans as required.
Charts nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to the care.
Completes accident/incident reports involving residents. Conducts follow up investigations. Submits appropriately.
Completes admission, discharge and transfer forms in accordance with established procedures.

Ensures that appropriate documentation concerning unauthorized discharges is entered in the resident's medical records in accordance with established procedures.
Provides and receives taped report and reviews 24 hr report daily.


Must possess a Nursing Degree/Diploma from an accredited college, university or school of nursing.
Must possess a current, unencumbered active license to practice as an RN in the state of Ohio.


Must have, as a minimum, one (1) year medical/surgical nursing experience.
Certification in specialty areas may be required.

Must be knowledgeable of laws, regulations and standards (Federal, State, CCAC, JCAHO, CARF, etc.) as related to job tasks performed.
Must possess appropriate patient assessment skills.
Must be able to read, write, speak, and understand the English language.
Must possess verbal and written communication skills.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
Must possess leadership ability and willingness to work harmoniously with and supervise other personnel.
Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing practices.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.
Must be knowledgeable of nursing and medical practice and procedures, as well as laws, regulations, and guidelines that pertain to Long Term Care and specialty areas as deemed necessary.
Must not pose a direct threat to the health or safety of other individuals in the work place.

Physical and Sensory Requirements
(With or Without the Aid of Mechanical Devices)

Must be able to move intermittently throughout the workday, including bending, lifting, standing, walking, sitting and possess manual dexterity.
Must be able to transfer residents with assistance; push wheelchairs and medication carts; lift and move supplies and records.
Must be able to cope with the mental and emotional stress of the position.
Must meet the general health requirements set forth by the policies of the facility, which includes a medical and physical examination.
